Beginning March 21, 2022, in alignment with the provincial government, the City of Hamilton has repealed its Face Coverings & Masks By-law and Physical Distancing By-law. As such, all City Recreation facilities and amenities no longer require patrons to wear a mask or face covering and physically distance.
However, the City still recommends that guests should not attend if they feel unwell, even if their symptoms are mild, and ask that anyone who is required to isolate does not attend. They encourage event organizers to create a list of guests so you can notify them if a guest develops symptoms of COVID-19.
So, FrancoFEST Hamilton has created this screening form in order to gather contacts from festivalgoers and contact them in case of an outbreak on the festival site. We respect your privacy and will commit to deleting all information given on this form after 15 days. Our intention is also to get your confirmation that you understand all risks linked to your presence at the festival, accept them and will not hold FrancoFEST responsible for consequences on your health.
Any records created as part of the screening may only be disclosed in accordance with the law.
